This sample document provides two templates that may be used for requesting a copy of an ACH Authorization: 

• from an ODFI to and RDFI, as well as
• from an Originator to an ODFI

Each template already starts with minimal pieces of information appropriate for a request, so that other party would be able to trace the transaction in a timely manner. These documents are also formatted to allow enough space on each page for you to make them fit the needs of your organization.
